You can use this form if it has been ** 14 days or less since the date of the Court's decisions **. You only have 14 days after the date of the Court’s decisions to file a Motion for Reconsideration and Modification.

The following are the reasons you can request these motions:

• the court’s reasoning that led to the current order was wrong,
• the court failed to consider evidence, or
• the judge should know about new information that affects the decision the court already made.

You may not use this form for the following reasons:

• to tell the judge about a real-world change of circumstances, or
• to tell the judge about a new argument that you are making for the first time.
