Affidavit of Indigency (fee waiver)

If you cannot afford to pay court fees or other costs related to your court case, this form will help you ask the court to waive the fees.

Depending on the kind of income you get, you may need to give information about your income, expenses, assets, and debts. You will also need to provide information about the case you are involved in.

This interview can make the Affidavit of Indigency and the Supplement to the Affidavit of Indigency form, if you need it.
