New Special Education Forms in Massachusetts

We're excited to announce that we have added four new interactive forms to Court Forms Online to help Massachusetts families and students with school and special education needs. These guided interviews were authored and approved by Massachusetts Advocates for Children (MAC), ensuring they meet the highest standards of advocacy and legal accuracy.
Our new forms make it easier for parents and advocates to navigate the educational system step-by-step:
- MAC Request for Independent Special Education Evaluation: If you disagree with a school district's evaluation of your child, this tool helps you formally request an Independent Educational Evaluation (IEE) at public expense.
- Request for a School Special Education Evaluation: This tool helps you generate a formal letter to your school district asking them to evaluate your child for an Individualized Education Program (IEP).
- MAC PRS Complaint (Language Access): A tool to file a Problem Resolution System (PRS) complaint with the Massachusetts Department of Elementary and Secondary Education (DESE) when a school fails to provide required translation or interpretation services.
- Request for Student Record: Assists parents and eligible students in exercising their right to request a complete copy of a student's educational record from their school.
