Before you start

Before you get started, gather your:

1. Docket number (case number)
2. Case Information (Court department, parties, county, court address)
3. Attorney's Information (address, phone number, email address)

When you are finished, you will need to:

1. Download the completed form
2. File the form with your court (instructions provided)
3. Deliver a copy of the form to the other party in your case or their attorney
